1. Submission of a nomination package, which includes:
a. Completed nomination form
b. Biographical sketch of the nominee
c. Executive summary, indicating why this candidate is deserving of the Campion College Alumni of Distinction Award, including how they demonstrate the key values of their Jesuit education in their professional and personal actions.
d. A release form signed by the nominee to:
i. Let his/her name stand in nomination; and
ii. Agree to attend the presentation ceremony if selected to receive the award
e. Additional supporting documents, such as a list of awards of public recognition, sample media reports, letters of reference, etc.
2. Completed nomination packages should be sent to the attention of:
